Dear Referee,

Please complete the Game Report Form below within 24 hours of your assigned game. Generally, only the assigned center referee (Ref 1) needs to complete the game report. However, anytime you encounter special circumstances (ejections, injuries, other notable incidents), we like to have all members of the referee crew submit individual reports. This will help us to get a better overall picture of any incident, and allow us to take the appropriate actions more swiftly. Please be as specific as you can - especially when it comes to cautions, ejections, injuries: always include player(s) name, team affiliation, reason(s) for your action, time of event(s), etc. Be aware - a number of fields do require data entry. If you accidentally leave a field blank, the design of this form will remind you to complete required fields. We appreciate your prompt feedback.
